$350 USD -- Residents, Fellows, Postdoctorates
$150 USD -- Predoctoral, BA, MS, RN
$100 USD -- RN educational sessions only
$30   USD -- RN day-rate; educational sessions only
$200 USD -- Additional family member
$200 USD -- Second additional family member

* The registration cost does not include hotel lodging. Please call the hotel directly or register online for lodging ($119 USD/night). Room block expires June 29, 2011.

** The registration cost includes course materials, breakfast and lunch each day, the Welcome Reception on Wednesday, July 20, Poster Reception on Thursday, July 21 and Dinner Banquet on Friday July 22.
The RN day-rate includes breakfast and lunch only.

Cancellation Policy

All cancellations must be received in writing before July 6, 2011. There will be a $100 USD administrative fee deducted from the refund. No refunds after this date.
